Documents needed

for the evaluation of foreign university education and qualifications as required in the Czech Republic

Overview

1. Application:

• The application is to be submitted to the Rector.The application must contain:
• First name and last name of the applicant, academic degree/s
• Date and place of birth (including the country)
• Identification document number (ID card or valid passport)
• Permanent address and mailing address (if not the same), e-mail address and phone number.
• Full name and address of school in foreign country
• Name of the programme and/or field of study
• Day of commencing and finishing study at the foreign University
• Reasons for applying
• Date and signature of applicant

2. Appendices to the application

• Declaration stating that the applicant has not obtained validation of the Diploma by any other university located in the Czech Republic and currently is not seeking validation by any other university located in the Czech Republic.
• Original or authorized copy of the Diploma issued by the foreign university
• Original or authorized copy of the Diploma Supplement
• Additional information confirming that the program of study concerned was provided by an institution approved to offer education equivalent to the tertiary education according the Higher Education Act and information about the contents of the study programme at the foreign university.

Unless the foreign university issued the Diploma Supplement, the applicant must provide an original document or a certified copy listing all programs and fields of study (courses) finished at the foreign university and including the workload and results achieved by the applicant.

All documents in the application submitted in a foreign language must be supported by a certified translation into the Czech language. This is not required if the validation is requested for continuous studies in the language that the documents have been issued in.
